The Opportunity 

We are seeking an experienced and dynamic Director of Care Management to lead our care management team, comprised of RN/SW Care Coordinators and non-clinical/clinical Care Navigators. As the Director of Care Management, you will be responsible for overseeing the development, implementation, and evaluation of care management programs and initiatives designed to ensure that Found members receive safe, high-quality, and coordinated obesity care.  In addition you will collaborate across the organization to design innovative approaches to that increase value, decrease cost, and deliver a best in class member experience.  The successful candidate will be passionate about leveraging technology in healthcare to increase access and to optimize the patient/clinician relationship. This is a challenging and rewarding position that requires strong leadership skills, attention to detail, and a commitment to excellence in care.

Primary Responsibilities:

• Oversee and develop the care management team to ensure that they are providing high-quality care management services to Found members
• Develop and implement care management policies and procedures to ensure optimal member outcomes and experience, industry standards, best practices, and compliance with regulatory requirements
• Establish and maintain relationships with providers, product, central operations and other stakeholders to ensure effective and efficient care coordination
• Collaborate with the interdisciplinary healthcare team to develop care plans that meet the needs of members, resolve barriers to care, and promote optimal outcomes
• Monitor and analyze data to evaluate the effectiveness of care management programs and services
• Identify areas for improvement and implement strategies to enhance the delivery of care management services
• Ensure that care management processes are efficient, effective, and member-centered
• Provide education and training to care management colleagues and providers on care coordination/navigation processes and best practices
• Participate in the development of the annual budget for the care management department and ensure that expenses are within budgetary constraints
• Collaborate across clinical operations to lead projects and initiatives as defined through the company objectives and key results 

Required Qualifications:

• Excellent interpersonal and communication skills.  Clinical leaders must have excellent communication skills, including the ability to deliver effective, actionable feedback, and build rapport with colleagues at all levels of the organization
• Ability to work professionally and effectively with the external stakeholders such as community providers, existing and potential business partners, and insurance companies as needed 
• Passionate about delivering safe, high-quality obesity healthcare, and harnessing technology to do so
• Strong, demonstrated aptitude for problem solving.  Clinical leaders must be able to identify and address barriers to care, such as financial or logistical challenges, and develop creative solutions to ensure members receive appropriate care.
• Demonstrated ability to prioritize and manage multiple tasks and projects at various stages of completion, and ensuring their successful completion
• Exceptional organizational abilities, with the ability to organize and prioritize competing priorities.
• Must be very comfortable and proficient using technology, including electronic health records and other specialized software

 

Minimum required education and experience:

• 2+ years of experience in a care operations, care management, or care coordination role 
• 2+ years providing direct patient care in an adult chronic disease management care setting such as primary care, cardiology, endocrinology, or an acute/critical  care setting such as Emergency Department or ICU care 
• 2+ years experience in leading within a clinical team
• Experience working as a part of an interdisciplinary care team 
• Clinical degree in Nursing, Nurse Practitioner, Physician Assistant, or Pharmacy

 

Ideal education and experience:

• 5+ years of experience in a care operations, care management, or care coordination role 
• 5+ years in an adult chronic disease management care setting such as primary care, cardiology, endocrinology, nephrology, or an acute/critical care setting such as Emergency Department or ICU care 
• 2+ years experience in leading a clinical team
• Experience working as a part of an interdisciplinary care team 
• Experience collaborating closely with a Product team or Product managers
• Clinical degree in Nursing, Nurse Practitioner, Physician Assistant, or Pharmacy;  plus experience in obesity medicine
• Certification such as Obesity Medicine Association (OMA), LEAN/SixSigma, Certified Care Manager (CCM), Accredited Case Manager (ACM) and Case Management Administrator Certification (CMAC)
